The aim of this prospective study was to evaluate the clinical application of the new double-armed robotic system (Da Vinci System, Intuitive Surgical, Sunnyvale, CA, USA) for endoscopic surgery. A Da Vinci surgical robot with two arms was used to create an access port for endoscopic surgery and create a double-armed working system in 30 patients with complicated and simple duodenal ulcer disease. The DuVinci was used to create an access port by using the Robotic Vision System, and a cautery was used for hemostasis. The surgical outcomes were evaluated. Successful duodenotomy was achieved in all patients in a mean (SD) time of 1.4 (0.7) min. The procedure was not found to be feasible in one patient because of the narrow angle of the duodenal bulb. The mean time for the closure of the perforation site was 10.9 (5.1) min. No major postoperative complications occurred. The mean operation time was 245.0 (56.1) min. The relatively large eccentricity of the colored regions suggests that the colors are also caused by scattering from the retinal pigment epithelium. Here, we use a novel psychophysical protocol with images of 3-D scatter structures to investigate the origin of the color in the central and paracentral visual fields. We establish a direct correspondence between the color of the spots and the scattering properties of the retinal pigment epithelium. Our results show that the spots are caused by the retinal pigment epithelium and most likely reflect the effects of light diffusion from retinal structures lying deep beneath the photoreceptors.
